Introduction: Health data in Iran are generated by numerous agencies, each with its own information systems, definitions and institutional logic. This fragmentation hampers the use of data in policymaking, service delivery and population health monitoring. This study aimed to identify the dimensions of intersectoral health data governance and to design a model suited to Iran's institutional context.
Methods: The study used a qualitative design and thematic analysis. Semi-structured interviews were conducted with 15 purposively selected experts in health policy and management, information technology, insurance and social security, and data governance. Theoretical saturation was reached at the thirteenth interview, and two additional interviews were conducted. Data were analyzed at three levels of basic, organizing and global themes using thematic networks; inter-coder agreement was 83%.
Results: 139 basic themes were grouped into 21 organizing and five global themes: integration and flow of intersectoral health data; institutional governance and accountability for data; trust, security and protection of data; access, sharing and responsible use; and national data-driven infrastructure. Ambiguity over data ownership and custodianship, the absence of shared meaning across systems, and the dependence of data exchange on administrative correspondence and personal relationships were key bottlenecks in data flow between agencies.
Conclusion: Iran's health data governance problem is more institutional than technical and stems from unclear rules on ownership, access, accountability and inter-agency trust. The proposed model can serve as a basis for designing a national health data governance mechanism and assessing agency readiness.
